Brief Cognitive Assessment Tool (BCAT)
The BCAT consists of screening tools that allow for an
accurate assessment of current cognitive and mood
functioning. Cognition is at the base of almost all aspects
of care and an accurate assessment is critical to effective
Offer an opportunity to address issues related to Caregiver
and Care Recipient concerns, such as:
- Caregiver stress
- Living arrangements
- Financial/Legal matters
One time in-home assessments
A comprehensive assessment in the care recipient's home which includes the items above plus:Cognitive assessments (BCAT)
- Home safety assessment
- Written summary of the face-to-face meeting
- A Plan of Care which includes a summary of the existing or potential problem areas, plus suggestions and resources for resolving these areas of concern.
(Clients implement, monitor and update the Plan of Care)
On-going Geriatric Care Management
Is the same as a one-time assessment. However, the GCM
remains part of the care team assisting with the implementation
and monitoring of the Plan of Care. The GCM also advocates
for Clients, accompanies seniors to appointments and is
available on an as-needed basis.
