At Family Eldercare Management, our goal is to help our clients achieve the best quality of life possible. We understand that one solution does not fit every situation so we take the time to listen and learn about each client's needs and preferences.
Understandably, most seniors prefer to stay in the familiar comfort of their own homes. Caregivers often find this choice difficult to manage. Our familiarity with the community and an extensive collection of local and national resources permits flexibility in tailoring care plans that maximizes the senior's independence and dignity.
We provide the following services:
- Referrals to community resources, explaining costs and monitoring services
- Intervening in crisis situations
- Coordinating care with hospitals and rehabilitation/nursing facilities
- Acting as liaison with physicians and other health care professionals
- Conducting home safety evaluations and recommending in-home care services
- Arranging for purchase and installation of durable medical equipment
- Identifying appropriate care facilities, assisting in selection and transition
- Arranging for enrollment in adult day care centers
- Arranging transportation to appointments and community services
- Providing assistance to long-distance caregivers
