Mission Statement:
Family Eldercare Management’s mission is to assist caregivers and care recipients in achieving and maintaining the best possible quality of life in the most cost effective manner. This is accomplished by conducting comprehensive assessments and providing a Plan of Care designed to meet your individual family situation. Recommendations are based on knowledge of senior issues, as well as, local and national resources. Monitoring and adjustment of the Plan of Care is available to meet your family’s changing needs.
What are Geriatric Care Managers:
Geriatric Care Managers – One of the best kept secrets in guiding others through the maze of caring for the disabled and seniors as they age.
Geriatric Care Managers (GCM) are social workers, nurses, psychologists and other health and human service professionals trained in issues related to aging and elder care. They provide assessments, resources, referrals, family mediation, education, advocacy and training.
The National Association of Geriatric Care Managers http://www.caremanager.org is the best way to locate a qualified GCM. Due to our knowledge of the aging process and the resources available, seniors are often able to safely live in their homes for longer at a higher functional level. This means lower healthcare costs through fewer hospitalizations, more efficient use of financial resources and less time lost from work to care for family members. Financial institutions, medical staff, attorneys, social service agencies and senior housing facilities also use GCM’s.
